The following letter was sent to all current patients in December 2020:
Dear Valued Patient:
After much thought and careful consideration, I have decided to close Intown Family Practice & Sports Medicine, PC (“IFPSM”), effective March 27, 2020. The last appointment date available at IFPSM will be March 20, 2020.
I will be joining Piedmont Physicians at Midtown at 1080 Peachtree St NE, Suite 12, Atlanta, GA 30309. Piedmont Healthcare is a healthcare system with whom I have shared the care of many of my patients. In addition to the resources of an excellent medical group, patients who elect to come with me will also enjoy some amenities we have not been able to offer in the past such as validated parking, acceptance of credit cards, and in-office lab draws, as well as easy access to the Midtown Marta station.
You have options for your future medical care, as set forth below. The option you choose is at your sole discretion.
I. You may remain a patient of Dr. Golusinski who will be practicing at Piedmont Physicians at Midtown at 1080 Peachtree Street NE, effective March 30, 2020. To make an appointment after that date with Dr. Golusinski, please call 404-253-3660. Should you choose to continue your health care with Dr. Golusinski, please complete the enclosed form for Piedmont and either fax it to (404) 719-5325 or email to IntownFPSM@aol.com. You may also call the Practice at (404) 577-7800 prior to March 1, 2020 to coordinate the transfer of a copy of your medical records to Piedmont Physicians at Midtown but please note the same form will need to be completed. We will be working closely with Piedmont to try to keep already scheduled appointments for those choosing this option and appreciate your patience during the transition period. Piedmont accepts the majority of insurance in which I currently participate. Two major exceptions to note: Piedmont does not participate with the Blue Cross Exchange (ACA) products and Aetna for Emory employees is non-par and would mean higher copays. We recommend you check your benefits and with your insurance as changes are constantly occurring.
II. You may choose another physician to continue with your medical care. If you are not acquainted with a physician, you may use your health insurance company’s provider directory. Once you have identified a new physician or practice, you may complete the generic medical release form and either fax it to (404) 719-5325 or email to IntownFPSM@aol.com. You may also contact us at (404) 577-7800 prior to March 1, 2020 to coordinate the transfer of a copy of your medical records. Again, the appropriate form will need to be completed.
Your medical records are confidential, and a copy can be transferred to another physician or released to you or another person you designate only with your written permission. For your convenience, we have enclosed a specific form for release of records to Piedmont and a generic Medical Records Release and Authorization Form for other purposes, and ask that you complete and return the appropriate copy to our office by no later than March 1, 2020.
If you do not make a written request to release or transfer a copy of your medical records by March 20, 2020, your records will remain with IFPSM who will serve as the custodian of such records. As custodian, IFPSM will retain such records as required by law and will comply with all applicable laws governing confidentiality, access, disclosure and fees. After that date you will need to request your records by fax to (404) 719-5325 or email to IntownFPSM@aol.com.
Pursuant to Georgia law (O.C.G.A. §31-33-3), medical record(s) retrieval, transmittal, certification or copying for reasons other than continuity of your medical care are subject to fee(s). These fees do not apply to records requests necessary to make or complete an application for a disability benefits program or vocation rehabilitation program. You may contact the Georgia Vocational Rehabilitation Agency at (678) 639-2100 for more details. Further, the State Board of Workers’ Compensation has instituted medical record retrieval rates that apply specifically to workers’ compensation cases and differ from fees noted above.
